the place, date, and identifying number, if any, of policy;
the names and addresses of the parties to the contract of insurance;
the names and addresses of the beneficiaries or, if they are not determined, a means to identify them;
the person, property or liability insured;
the nature of the risks insured;
the amount of the coverage;
the amount, time and manner of payment of the premiums;
the time from which the risks are covered and the term of the coverage;
territorial limits, if any, of the coverage; and
the law applicable to the contract of insurance.
The insurance policy may contain other particulars.
