the name and the date of birth of the insured person;
the name of the beneficiary or a means to identify him;
the occurrence on which the payment of the agreed amount depends;
the manner of calculating any reduction in value of the policy;
the manner of calculating the surrender value of the policy and the rights relating to the surrender value of or advances on the policy;
the conditions of reinstatement of a policy; and
the conditions of conversion of the policy, if applicable.
