the day, month and year of the death;
the name, date and place of birth of the deceased;
the names, dates and places of birth of the father and mother of the deceased;
the name, date and place of birth of the spouse of the deceased if such spouse is still alive, and the date of the marriage; and
where appropriate, the name, date and place of birth of the person who makes the declaration of the death.
