Article 366
Contents of Memorandum of Appeal
the name of the court which gave the judgment appealed from, the date of such judgment and the number of the suit in which it was rendered;
#Attached to the memorandum of appeal there shall be a certified copy of the full record of the proceedings in which the judgment appealed from was delivered and of such judgment.
#The appellant shall state whether he bases his appeal entirely on the record of the original hearing and shall, where appropriate, attach to the memorandum of appeal an application for permission to call additional evidence, stating:
#the reasons why such evidence was not produced in the court which gave the judgment appealed from, and why it should be produced in the appellate court.
#The appellant and the respondent shall respectively provide sufficient copies of the memorandum of appeal for service on each of the other parties.
