Article 89
Unknown or Uncertain Information
(1)
If one of the blank spaces left in a record of civil status cannot be filled in on account of lack of sufficiently accurate information given to the person who keeps the registers, such person shall fill in the blank space by inserting the word “unknown”.

If the civil status officer knows approximately the particular to be entered, he shall enter such particular in the appropriate place adding the word "probably".
