Particulars of Record
The record of birth shall show:
#(a)the day, month and year of the birth;
# (b)the sex of the child;
# (c)the name which is given to him;
# (d)the names, dates and places of birth of his father and mother;
# (e)where appropriate, the name, date and place of birth of the person making the declaration.
